Community Outreach and Family Support Services
Provides low to moderate-income residents access and referrals to financial, social, health and legal resources with a focus on families with children. Clients are encouraged to take the initiative with regards to self advocacy, including phone calls, appointments and follow ups.
In order to best serve clients, an appointment is necessary.
Public Assistance may be accessed online through the Virtual Gateway by appointment only.
Items needed for your application are:
• Mass Health: birth certificates, picture ID, verification of income for 2 weeks
• SNAP (Food Stamps): verification of income for the past 4 weeks, Social Security numbers and birth dates of all household members
• WIC
• Veterans Benefits
• Long Term Care
Fuel Assistance
Items needed for your application are:
• Picture ID for head of household
• Birth dates and SS number for all household members
• 4 weeks of verification of all income from all sources
• Landlords-name/address/telephone number
• Verification of rent/mortgage
• Verification of ownership-(RE taxes/mortgage)
• Utility bills
